phone insurance??

How much extra is it? I can't find it on the Verizon website, if someone can post a link to it it would be great.

4.99 a month, $50 deductiable, 2 deductiables per year, has to be added within 15 days from time the phone was activated

Just type insurance into the search on the website it should give you various links.
