have not heard of them actually sending a replacement battery under warranty. I have heard them selling the customer a new one 25% off. I am not sure if they would even warranty a battery within the 15 day period of getting a brand new phone...

100% Coverage 0-3 months
75% Coverage 4-6 months
50% Coverage 7-9 months
25% Coverage 9-12 months

This is only available in stores if you are within 60 miles of one and they have to test the battery first. Take the phone in with a low battery to start with. They will drain it with a call to another VZW phone then charge it and drain it again. If the second call isn't around 80% of the stated usage time it's deemed defective.

batteries are warrantied by manufacturer for 12 months.as long as charge on holds 80% or less of what it should be, you can get it replaced. i have done it at store a few times. tried to tell me warranty isnt for 12 months. so had to show them instruction book that comes with phones. even showed them the manufacturers web sites on it. so if you go in and battery is only holding 80% or less charge they have to replace under warranty.
