i have a question about a rebate

i bought a phone and have to mail in the rebate information. What all do i need to mail off to receive the rebate?

The rebate form, copy of the reciept, and the ORIGINAL barcode label off the side of the box

I had 2 different rebate forms in the past, and one wanted a copy of the barcode label, the other form wanted the original.
