The latter. But it only matters to you if you're on a preffered nation plan. Home plans, stay in your state, nation plans, stay in the USA.

cingular extend basicly means you are roaming off the Cingular Network..... if you are on a (new)Cingular Nation plan.. dont worry about it.. as long as your phone has a signal you can make a call.. no roaming.

Cingular Extend just means you are on another carriers tower or your IRDB needs to be updated. Never follow "extend" as a guideline for roaming charges! Always follow your calling map.
GSM Nation plans have no roaming charges even if "extend".
Region plans have no roaming charges IN THE REGION ONLY even if "extend".
The only time you have to worry about "extend" is if you are on a "preferred nation" plan.
