Yup. Must be redeemed by 7/31. Must have been purchased by 6/4. When you redeem the tickets, you must put in a valid mobile number, valid SE IMEI and valid purchase date.

Yup, my own name and my own mobile number. My guess is they only validate the IMEI and dates since the offer does not require new service activation but only a purchase of a SE handset.
