Call T-Mobile Customer Care and find out if you're eligible to have it unlocked.

Or you can do a web search for the instructions. When I unlocked mine I think it was around $85 for the software and instructions.

Calling Customer Care is better since T-Mobile, at this time, doesn't charge anything for unlocking a handset. The Sidekick handsets are the only ones they won't unlock.
